As a health care provider, there are 3 forms of consent which we need to have from you.

Consent to hold and maintain records under the UK General Data Protection Regulation (UK GDPR)

Consent to process and/or share information for anything other than the above
Everyone is entitled to access health care and for us to work with you we only need your consent – not your parent or carer.
However, you may wish for your parent/carer to be involved – that is your decision. We will respect your confidentiality and uphold your privacy, but there may be times when we will need to speak to someone else who supports you.
